Prenuvo-powered study published in Nature Journal links smoking to brain shrinkage

AI-powered MRI study finds link between smoking and early signs of brain aging in 10,000+ healthy adults

Redwood City, CA — [July 23] — Prenuvo, the leading whole body MRI for proactive health screening, today announced its research has been published in Nature Partner Journal – Dementia (npj-Dementia). The research reveals a strong association between cigarette smoking and brain atrophy in a cohort of 10,134 healthy adults scanned using Prenuvo’s full-body MRI platform. Led by Dr. Somayeh Meysami, MD, Assistant Professor of Neurosciences at Saint John’s Cancer Institute and Clinical Research Scientist at the Pacific Brain Health Center, Pacific Neuroscience Institute (PNI), the study utilized advanced structural MRI and deep learning segmentation to show how smoking is linked to regions of the brain commonly affected in Alzheimer’s disease.

The full list of collaborators include: Somayeh Meysami, Saurabh Garg, Sam Hashemi, Nasrin Akbari, Ahmed Gouda, Yosef Gavriel Chodakiewitz, Thanh Duc Nguyen, Rajpaul Attariwala, Kellyann Niotis, David A. Merrill, and Cyrus A. Raji.

The research found that individuals with any history of smoking (measured by pack-years) had notably lower gray and white matter volumes compared to non-smokers. Greater cumulative exposure (more pack-years) significantly predicted volume reductions in key brain regions, including the temporal lobe, parietal lobe, hippocampus, precuneus, and posterior cingulate. The analysis also highlighted body mass index (BMI) as a contributing factor, suggesting that higher BMI may amplify the negative impact of smoking on brain structure.

“Our results clearly show that smoking significantly impacts brain health by its link to increased brain atrophy in areas critical to cognitive function,” said Dr. Somayeh Meysami, MD, lead author of the study. “These findings reinforce the urgent need for preventive measures addressing both smoking cessation and healthy weight management.”

Senior author Cyrus A. Raji, MD, PhD, Associate Professor of Radiology at Washington University in St. Louis added, “This work underscores the interconnected nature of lifestyle factors like smoking and obesity in accelerating brain aging. Understanding these relationships provides an essential foundation for developing targeted strategies aimed at preventing dementia and preserving cognitive health.”

The study’s design, utilizing a large sample size and sophisticated neuroimaging, makes a significant contribution to public health by identifying modifiable risk factors for cognitive decline and dementia. Future research will further explore how interventions targeting smoking and BMI might mitigate or even reverse these adverse effects.

“This is exactly the kind of insight that wasn’t measurable before. By combining high-fidelity imaging with the largest dataset of healthy brain scans and intelligent, accurate, and reproducible measurement of small brain structure using AI, we’re now able to quantify subtle but critical effects – like smoking-induced brain atrophy – with unprecedented clarity,” said Sam Hashemi, Vice President of Artificial Intelligence & Research at Prenuvo. “It’s a window into what becomes possible when AI meets population-scale precision imaging, opening the door to earlier detection, targeted prevention, and a deeper understanding of cognitive aging.”

Prenuvo-enabled research wins 2025 Journal of Alzheimer’s Disease Alzheimer Award

Groundbreaking research using Prenuvo whole body MRI data earns the Alzheimer Award, awarded to Dr. Cyrus Raji by the Journal of Alzheimer’s Disease

SAN FRANCISCO July 8, 2025 Prenuvo, the leading whole body MRI for proactive health screening, today announced research powered by its imaging dataset has received the prestigious 2025 Alzheimer Award from the Journal of Alzheimer’s Disease (JAD). The award, granted for the last 25 years, honors one of the most impactful studies advancing the field of Alzheimer’s research in the past year and was selected by the international JAD Editorial board out of 800 submissions. 

The awarded study, “Exercise-Related Physical Activity Relates to Brain Volumes in 10,125 Individuals,” found that just 25 minutes of moderate to vigorous physical activity per week is linked to greater brain volume in regions vulnerable to Alzheimer’s, supporting exercise as a preventive strategy for dementia. The study used Prenuvo’s extensive normative dataset of proactive whole body MRI scans to identify structural brain changes in asymptomatic individuals that may precede clinical signs of Alzheimer’s. The study marks a significant step toward earlier, noninvasive detection of the neurodegenerative disease.

“Our findings suggest that subtle brain structure changes linked to Alzheimer’s may be detectable earlier than previously believed, especially with access to high-resolution imaging and AI tools that can segment and analyze these small structures at scale,” said lead study author Dr. Cyrus Raji, PhD, Mallinckrodt Institute of Radiology, Washington University in St. Louis. “This research was facilitated by Prenuvo’s unique MRI dataset, which combines whole body imaging with high spatial resolution, enabling a precise level of structural detail.”

While traditional Alzheimer's diagnostics rely on PET scans and cerebrospinal fluid analysis, this study highlights how non-invasive, whole body MRI, paired with advanced analysis using AI, can reveal early indicators of cognitive decline in large, diverse populations. Prenuvo's anonymized imaging data allowed researchers to examine the structural brain correlates of Alzheimer’s in people without clinical symptoms, helping to push the field of early detection forward.

The award includes a bronze medal featuring Alois Alzheimer and a $7,500 honorarium to be presented at the Alzheimer’s Association International Conference in Toronto, Canada from July 27-31st, 2025.

What our latest AI-assisted MRI research shows about alcohol, depression, and early detection of mental health disorders

At Prenuvo, we believe that answers to today’s most pressing health challenges are already inside us, if we know where to look. Using our proprietary dataset, which is the largest collection of whole-body MRI scans in primarily asymptomatic individuals in the world, our in-house AI research team and research partners are releasing two new studies that explore how everyday habits leave structural footprints in our bodies and brains.

Both studies have been presented at the 2025 ISMRM Annual Meeting (International Society of Magnetic Resonance).

Here’s what we found:

1. Just 2 drinks a day? Your brain and organs already know

While much of the research out there on alcohol focuses on those with alcohol use disorder, this study looked at people who drink moderately (two or more drinks a day) and found that even without clinical addiction, the effects are measurable and systemic.

In a study of over 1,100 people with no diagnosis of alcohol use disorder, moderate drinkers showed:

  • Loss of brain volume in regions tied to memory, motor control, and decision-making
  • Liver volumes 10% larger than non-drinkers
  • Visceral fat levels 15.6% higher than non-drinkers

The most surprising part? Changes in organ size were independently linked to brain atrophy, suggesting that alcohol’s effects aren’t siloed; they ripple across systems. This points to a broader, systemic toll that moderate drinking may take on long-term brain health. Learn more: link to full study.

2. Depression reduces brain volume, but exercise may help reverse it

Exercise is widely recommended to improve physical and mental health, but what’s actually happening in the brain when we move our bodies? A large-scale AI-assisted MRI study of 6,458 individuals (1,579 with depression) showed that individuals with depression had significantly smaller brain volumes than those without. However, among those who did exercise, some of that volume was preserved or even increased.

In depressed individuals, exercise was most strongly linked to brain growth in:

  • Medial orbitofrontal cortex: Involved in reward, emotion, and decision-making; more volume here may help regulate mood and motivation.
  • Lingual cortex: Associated with visual memory and emotional processing.
  • Rostral middle frontal cortex and middle temporal cortex: Critical for attention, social reasoning, and interpreting emotional cues.

In non-depressed individuals, exercise showed the greatest impact on:

  • Thalamus: The brain’s “relay center” that helps route information between regions.
  • Fusiform cortex: Important for facial recognition and visual memory.
  • Cerebellum: Known for motor control, but also increasingly recognized for its role in mood and cognition.

This suggests that not only can exercise mitigate the structural brain impacts of depression, but the brain responds differently depending on mental health history. It's a powerful clue in understanding how, and for whom, lifestyle interventions might have the greatest neuroprotective impact. Learn more: link to full study.
